Dear Sir,

Have you received our email regarding form of registration to be filled in and signed? If you haven't, we would send one and please return it immediately. We want to know also your complete name and address.

As we know that the quiz is held in cooperation with Indonesian Ministry of Tourism and Creative Economy and the followings are several requirements that should be met before and after taking the trip to Indonesia:

1. All the winners have to fill in and sign the attached form of registration which certifies you as the winners of Wonderful Indonesia Quiz 2012 and send it back to us by attaching copies of your passports and letter of recommendation from Indonesian Embassy or Indonesian Consulate General at respective countries.

2. Upon returning to respective countries, all the winners have to give testimonies about their experiences while taking the trip to Indonesia either in written or in video and send it to www.indonesia.travel The testimony will be published at www.indonesia.travel.

Here with we attach the form of registration from Indonesian Ministry of Tourism and Creative Economy. Please fill in the form and sign it and send it back to us immediately.
